Healthy North Coast’s Healthy Living and Ageing team has launched the first Regional Aged Care Connect meetings, a new forum bringing together residential aged care services and health partners to strengthen local communication and collaboration.
Two inaugural meetings were held:
- One in Port Macquarie, hosted by RSL LifeCare and chaired by HNC’s Jane Moye
- And in Grafton, hosted by South Grafton Community Aged Care and chaired by HNC’s Jo Winwood.
Both meetings welcomed representatives from local residential aged care homes, hospitals, Aged Care Outreach Services (ACOS) and NSW Ambulance. The strong attendance reflected a shared commitment to improving care coordination across the region.
Discussions were highly engaged and solution-focused. The ACOS team from both regions provided updates on key service developments, including enhanced transition support for residents returning to care homes, as well as insights into Hospital in the Home and its role in supporting aged care residents.
Residential aged care representatives also shared valuable perspectives from day-to-day practice, highlighting opportunities to strengthen communication and information sharing.
Regional Aged Care Connect Forum
The Regional Aged Care Connect forum is designed to support relationship building and shared local problem-solving, with a strong focus on improving transitions between hospital and residential aged care, specific to each local community. Details below:
