As we all know chronic disease impacts everyone involved in managing our populations health. The Australian Institute of Health and Welfare estimated that in 2021 almost half of us lived with one or more chronic conditions.
These long-term conditions continue to pose a significant burden on individuals, families, North Coast communities and the healthcare system. The impact of chronic diseases extends beyond physical health, affecting mental well-being and overall quality of life.
