Chronic Disease impacts everyone involved in managing our populations health. The Australian Institute of Health and Welfare estimated that in 2021 almost half of us lived with one or more chronic conditions.
These long-term conditions continue to pose a significant burden on individuals, families, North Coast communities and the healthcare system. The impact of chronic diseases extends beyond physical health, affecting mental well-being and overall quality of life.
GPs, Nurses, Allied Health Professionals and Practice Managers care for the health of North Coast communities by managing chronic diseases in their practice everyday.

Nigel Cronin is the Associate Director for Primary Care Programs at Healthy North Coast and has a wealth of experience in education, training, management and clinical nursing. He previously worked for Sydney Local Health District, commencing as a registered nurse in ICU and then as a Clinical Programs Manager and Workforce Manager, which also saw him develop management and leadership programs. Nigel has also taught at both the University of Sydney and University of Tasmania. He has a strong passion for education and workforce development as he sees this as the cornerstone for both good patient health outcomes, and, for primary health care employee work satisfaction.

A/P Kevin Alford dedicated 44 years of his medical carer to the people of the Mid North Coast of NSW. He has been a clinical specialist, a mentor, an educator and an innovator for cardiology in the region. Now in his well deserved retirement he will once more return to our region to lead a discussion around heart failure and its management.

Jannie Denyer is a Clinical Nurse Specialist 2 and the Cardiac Rehabilitation Coordinator. Jannie originally set up the Cardiac Rehabitiation service in Port Macquarie in 2004, offering a multidisciplinary team approach to supporting cardiac patients in their recovery from surgery, cardiac stenting and will review referrals of other cardiac issues. The program provides support, exercise and education that’s led by health professionals and is personalised to strengthen heart function. It helps the patient make long term lifestyle changes for a healthier life.

Kelly Eyeington graduated with a Batchelor of Social Work (Hons) in 2004 and has been a Senior Social Worker at the Mid North Coast Brain Injury Rehabilitation Service for 5 years and is the Social Work Head of Department at Port Macquarie Base Hospital. Prior to working at PMBH and MNCBIRS, Kelly worked as a Sexual Assault Counsellor in Community Health, and for Correctional Services in the Northern Territory and NSW as a Probation and Parole Officer.
Katie Harris graduated with a Bachelor of International Development Studies in 2018. She went on to complete a Masters of Social Work in 2021. She has been working at Port Macquarie Base Hospital as a Generalist Hospital Social Worker since 2021 and more recently has been the designated Social Worker for the MAU/3D wards. In her role as the 3D/MAU Social Worker, she is predominantly working with medically acute patients presenting with stroke, cardiac and pulmonary diseases and the psychosocial complexities associated with such presentations. Katie has a strong focus on advocacy in her work and is dedicated to fostering care and connection with the people she works with.
Ingrid Sivertsen is a dietitian and the MNCLHD’s District Manager for Integrated Primary Care. Ingrid has worked as a dietitian in a busy multidisciplinary allied health practice on the Mid North Coast. She currently manages the Enhancing Community Care allied health team, along with the Bowra GP Clinic and integrated care projects team for the MNCLHD. Ingrid has a keen interest in promoting the role of allied health clinicians, and in advocating for integration of care between services and disciplines for the benefit of our community.
Phil Rudd is a senior Community physiotherapist and coordinator of the Osteoporosis Clinic at Port Macquarie Base Hospital. As part of his community physiotherapy role he runs the Pulmonary Rehabilitation program which provides reconditioning exercise and health management education for patients with chronic respiratory disease.

Dr Mara was the inaugural Director of General Practice at the Federal AMA. Rocco has over 20 years experience in the health industry and gained a variety of experience across the health services continuum in both NSW and QLD public and private sectors. Rocco has has a special interest in rehabilitation and client centred approaches to chronic disease management and service design. Currently Rocco is the Respiratory Services Mmanager for Northern NSW Local Health District, actively involved with the development of services to improve quality of life for people with chronic respiratory diseases.
Matt is a dedicated Exercise Physiologist and Outpatient Rehabilitation Lead at the Wesley Hospital. With over 10 years of experience in the healthcare industry, he has established himself as a trusted expert in his field. In his role as Outpatient Rehabilitation Lead, Matt is responsible for overseeing the rehabilitation programs of patients recovering from various injuries and illnesses. He works closely with a team of Allied Health professionals and specialists to develop personalised treatment plans that address the specific needs of each patient.

Kim Welch (CCRN, RN CDE) has worked extensively across Community health, General Practice and Private Practice over the past 20 years, with a focus on the prevention and management of diabetes, heart disease and stroke. Previously working in Remote Area Nursing, Emergency, ICU. and a Facilitator and Provider of the Life! Program (prevention of diabetes, heart disease and stroke). With a team of Facilitators, she runs around 30 programs a year for general communities, GP Practices, workplace and CALD communities. Kim finds great fulfilment in giving people the education and the tools to support and make changes to their lifestyle.

Emma aims to improve a persons’/carers self-management knowledge with a goal of fostering a good relationship with food working with an individual’s own health care goals.
Michelle is the Clinical Nurse Consultant for Diabetes across NNSWLHD, a role that encompasses clinical leadership and strategic planning. Michelle started nursing in 1995, and developed an interest in diabetes after completing a hospital based diabetes workshop at Royal Darwin Hospital in 1998. After working in various nursing management roles, she completed her Grad Cert in Diabetes, became credentialed and has been working full-time in diabetes across all age groups since 2006. Michelle’s goal is to work towards improving the patient journey for persons living with diabetes.

After owning/ managing gymnasiums for 10 years she trained again to become an Allied Health Physiotherapy Assistant. In this capacity she now works at the Wellness Centre in Ballina as a Lungs-In-Action Instructor focusing on the over 65 years client group and working with a team of Exercise Physiologists.

Dr Fagan is a UK trained Gastroenterologist who moved to Lismore Base Hospital in June 2022. He is passionate about liver disease and is keen to help develop services in the Northern NSW LHD.
After completing his undergraduate training and junior doctor training in the Wessex Deanery, England Kevin completed his Doctor of Medicine on “Biomarkers in the assessment and management of patients with chronic liver disease”, with Professor Elizabeth Powell at PAH, Brisbane during his specialist Gastroenterology training. Returning to the UK to complete his training including 13 months at the Royal Edinburgh Infirmary where he completed his liver transplant training. He took up his first Consultant post at the University Hospitals Plymouth in 2017 as a Consultant Hepatologist, during which he led the team to achieve Improving Quality in Liver disease (IQILS) accreditation.
