The Aged Care Onsite Pharmacists (ACOP) Program offers a practical, hands on solution by placing a credentialed pharmacist directly in your home. Funded by the Australian Government, ACOP embeds a pharmacist as part of your clinical team. They work alongside nurses, care staff and GPs to review medications, support safer practices, contribute to clinical governance and help build a culture of continuous improvement. Homes already participating report stronger teamwork and more resident centred care.

A note on eligibility
From 1 February 2026, aged care homes no longer need an eNRMC system to participate in the ACOP program. This change gives providers greater flexibility in how they manage medication processes and allowing full access to onsite pharmacy support.
Credentialling changes for ACOP pharmacists:
- Pharmacists with existing Medication Management Review (MMR) credentials may continue in the ACOP roles until 30 June 2026.
- From 1 July 2026, all ACOP pharmacists must complete an Australian Pharmacy Council accredited ACOP training program or meet requirements through an approved Recognition of Prior Learning pathway.
These updates ensure consistent, high quality pharmacy services within residential aged care homes.
