My Health Record (MHR) is becoming a core digital tool in residential aged care, helping teams deliver safer clinical care, smoother transitions and stronger alignment with aged care reforms and the Quality Standards.
If your home isn’t yet using MHR, speak with your head office or digital team about enabling access or integrating it into your clinical software.
How My Health Record supports better care for residents
My Health Record is especially useful during hospital transfers, after-hours care, resident onboarding and when new staff are caring for residents they’re not familiar with.
It provides access to essential clinical information including:
- discharge summaries and current medication list for safe medicines management and continuity of care
- Residential Care Transfer documents to support consistent, reliable handovers
- advance care plans and goals of care, ensuring residents directives guide decisions
- pathology and imaging results to reduce repeat testing and inform clinical decisions
- vaccination history, now visible to aged care providers for safer onboarding and infection prevention.
Supporting reform and Aged Care Quality Standards
The Royal Commission recommended universal use of My Health Record to strengthen information sharing and continuity of care. MHR also supports compliance with the Aged Care Act 2024, including Standard 3(3)(e), which requires advance care plans, medications and clinical information to be available to hospitals and other health professionals.
Using MHR improves communication, supports safer care and reduces administrative burden.
