An Advance Care Directive (ACD) is a legally binding record of decisions or statements that describes a person’s future preferences relating to medical treatment, to inform healthcare decisions, when a person loses capacity.
Supporting a patient to develop an Advance Care Directive ensures they receive patient-centred care that aligns with their values and wishes, when they need it the most.
Why upload to My Health Record?
Uploading a patients ACD onto their My Health Record is a vital step to ensure patients receive care according to their preferences when they present to the emergency department.
For users of Best Practice software, ACDs can be uploaded directly with the patient’s consent. Clinicians using other practice software can provide instructions to patients so they can upload the document themselves or with support of a carer.
Dr Thomas Fursdon, ED consultant, emphasises:
“My Health Record is our only source of information for a patient when they come in sick in the middle of the night. Having access to an advance care directive is probably the single biggest thing that could change our care for patients in the emergency department.”
Updated HealthPathways resources
The Advance Care Planning HealthPathway has recently been updated to include:
- step-by-step instructions for uploading ACDs to My Health Record using Best Practice software or by a patient
- links to a wide range of patient resources on advance care planning
- culturally appropriate materials for Aboriginal and Torres Strait Islander peoples, and those from culturally and linguistically diverse backgrounds

- guidance on how to initiate conversations about advance care planning with patients.
