With not long to go, excitement is building for the Chronic and Complex Care Forum 2025, taking place on Saturday, 31 May at Pacific Bay Resort, Coffs Harbour.
This full-day event brings you, our local primary care providers, together to explore the latest in chronic and complex care. Designed for you, the forum offers a rich program of expert-led sessions, practical case studies, and valuable networking opportunities.
What’s on the program?
You will gain insights and practical strategies across a range of key topics, including:
Aboriginal and Torres Strait Islander health:
Aboriginal and Torres Strait Islander people make up around 7% of the North Coast population—well above the state and national average. This session explores culturally centred approaches to care, the impact of historical trauma on chronic conditions, and the importance of cultural connection in improving health outcomes. Susan Parker-Pavlovic, a proud Yaegl woman and Director of Aboriginal Health Partnerships at Healthy North Coast, will share insights alongside Healthy North Coast GP Medical Educator, Dr Chris Bentley on how to better support Aboriginal people throughout their health journey.
Palliative and end-of-life care:
With an ageing population, demand for palliative and end-of-life care on the North Coast is rising. This interactive session will feature a panel of clinicians, policy experts, and sector leaders—including Dr Chris Bentley, Kirsty Blades (CEO, Palliative Care NSW), Dr Sarah Mollard (GP & HealthPathways Clinical Lead and Mary Tullipan (HealthPathways Clinical Editor, Pharmacy) – discussing early intervention, care coordination, and practical tools to support patients and families. Key updates include the new Support at Home package and strategies to improve outcomes through integrated, person-centred care.
Burnout & self care:
Dr Kathryn Hutt, GP and Medical Director of Doctors’ Health NSW, brings deep experience in supporting the wellbeing of doctors and medical students across the state. In this interactive session, she will guide participants through a reflective exploration of the barriers to self-care in clinical practice and introduce practical tools – like a personalised ‘bushfire plan’ – to help clinicians recognise early signs of burnout and take proactive steps toward a more sustainable, values-aligned career.
Alcohol and Other Drugs:
The North Coast has the highest rate of illicit drug use in Australia, with 23% reporting use in the past year. This session will explore the neurobiology of dependence, managing substance use disorders – particularly alongside anxiety and chronic pain – and recent advances in addiction medicine. Dr Buddhima Lokuge, a dual-qualified addiction and public health physician, clinical researcher, educator and author will share practical strategies for primary care.
Gender-affirming care:
The Healthy North Coast Health Needs Assessment highlights persistent barriers to safe, inclusive care for LGBTQ+ communities in the region. This session will introduce the foundations of gender-affirming care, including inclusive consultations, key health disparities, and primary care strategies such as psychosocial support, hormone therapy, and referral pathways. Dr Nicola Holmes will guide participants through practical approaches to delivering respectful, person-centred care that supports the wellbeing of transgender and gender diverse patients.
Obesity and diabetes:
With 73.5% of the Mid North Coast population being overweight or obese, well above the national average of 66.5%, obesity is a key driver for chronic diseases in our area. Dr Katharine Gupta, consultant endocrinologist, will share her views on obesity as a disease and discuss practical strategies focussing on the role of pharmacotherapy in managing obesity in primary care. With a strong interest in diabetes in pregnancy and a passion for collaborative care, Dr Gupta brings valuable insights from her experience in joint GP and specialist case conferencing.

The forum will focus on the drivers of complexity underlying chronic disease management in primary care, improving health equity, and integrating care. This will be explored through engaging presentations, expert panels, and interactive discussions designed to inspire collaboration and knowledge-sharing.
