Optimising the care of residential aged care home (RACH) residents across our region is a priority for Healthy North Coast and our Healthy Living and Ageing team.
The team has implemented several initiatives to support RACH residents to receive the right care in the right place at the right time. By utilising the supports available, GP time can be optimised and utilised for those who need it most.
Initiatives include:
- Implementing a fully funded GP telehealth service for RACHs;
- Equipping RACHs with video-telehealth equipment for virtual consultations;
- The development of a clinical toolkit for management of deteriorating residents.
Video-telehealth equipment
We are pleased to be able to announce that participating RACHs in the Healthy North Coast footprint are now enabled with video-telehealth equipment to enable remote consultations.
As part of our commitment to advancing telehealth in our region, a number of RACHs are participating in a pilot program to trial the Visionflex Telehealth Cart, in which remote diagnostic equipment (e.g. ECGs, stethoscopes) allows for a comprehensive assessment to be undertaken. Early feedback from pilot participants has shown a promising number of GPs and specialist services embracing the equipment.
HNC has also invested in a self-paced education program of short, ‘micro-learning’ modules, to increase awareness of the fundamentals of setting up, conducting or participating in a video-telehealth consultation. This will be available to RACH staff, GPs, allied health and specialist services from early 2024.
Deteriorating Resident Triage Tool
Together with a specialist geriatrician, Healthy North Coast has developed a triage tool for management of a deteriorating resident, with the objective being to minimise unnecessary hospitalisations. This tool will be piloted with a small number of RACHs in early 2024, with region-wide implementation planned for mid-2024.
Integrated Model of Care
The HNC Healthy Living and Ageing team are working toward regional forums in early 2024 to co-design solutions for delivering the right care, in the right place, at the right time for RACH residents. A consultant with extensive experience in integrated care is being sourced to develop models that can be adapted in towns throughout the footprint. All key stakeholders – RACHs, GPs, LHDs, NSWAS and community pharmacies – will be invited to attend the round table discussions.
