The Get Healthy service is a free, state-wide phone or online health coaching and support service, currently delivered by Diabetes Australia and funded through NSW Health.
Get Healthy’s new healthy ageing support went live on 19 May, and aims to support older adults to eat well, be more active and prevent falls – the leading cause of hospital presentations in this age group.
The new healthy ageing support is based on a Randomised Control Trial which showed that regular telephone-based health coaching over a 10-week period was well accepted and achieved significant improvements in nutrition and physical activity knowledge, confidence and wellbeing, compared with providing information only
All participants aged 65 years or older, or 50 years and older if Aboriginal, will be offered the following healthy ageing support (younger people can also receive support):
- Up to 10 health coaching sessions by qualified health professional coaches (dietitians, exercise physiologists and nurses). Coaches work with each participant to set and achieve realistic healthy eating and active living goals to reduce the risk of chronic disease, maintain wellness and independence.
- Participants are also screened for falls risk and are offered a suite of age-specific resources, including at-home exercise circuits, information related to bone health, bladder and bowel health, oral and eye health, and falls prevention.
Patients can self-refer or can be referred by their clinician through the website or general practice medical software.
