MBS changes for GPs take effect from 1 November 2018.
Spirometry Fees
The existing MBS item no. 11506 for spirometry testing attracts an MBS fee of $20.55 for GPs and can be claimed as often as needed throughout the year.
An additional item, no. 11505, is being introduced with a higher fee of $41.10. The conditions for claiming the new item are more stringent: three or more recordings have to be made in the one consultation, and the item number can only be charged once a year.
The introduction of the additional item is to allow GPs to conduct more rigorous diagnostic testing once a year, and then monitor the patient’s condition under the existing MBS item by conducting the simpler testing as required throughout the year. This is one of the few recommendations made by the MBS Review Taskforce that has so far seen an increase in MBS funding for doctors.
Focussed Psychological Strategies Allowed by Telehealth in Certain Remote Areas
Telehealth consultations can be conducted by appropriately trained GPs if the patient and provider are both in the Modified Monash areas 4-7, and the patient is unable to attend the practice. There are a number of conditions that need to be met to claim items nos. 2721 to 2727, so please check this link.
Direct Referrals for Sleep Disorders
Providing that a GP uses an approved assessment tool (see below), they will be able to directly refer patients for obstructive sleep apnoea studies. The new referral process also allows referrals to be made to a business, and not just to a specific specialist or physician, and also allows a specialist to on-refer to another specialist if he is unable to accept the referral.
Patients must be assessed using one of the following tools, reach the scores set out below and have an Epworth Sleepiness Scale of 8 or more for the direct referral to be made.
- STOP-BANG score of 4 or more
- OSA50 score of 5 or more
- Berlin Questionnaire – high risk
If the above criteria is not met, a referral to a sleep or respiratory physician must be made for further assessment.
