Over the past 8 weeks, Healthy North Coast was thrilled to be joined by around 50 professionals to brainstorm the barriers and enablers in joining care systems to support residents in aged care to receive quality care at the right time and the right place and avoid unnecessary hospitalisation.
There was representation from GPs, RACHs, nurse practitioners, ambulance, pharmacy and hospitals in a series of co-design workshops and playback sessions.
Integrated support
Supplemented by global research on best practice and a look at aligned models elsewhere, the result is a ‘hot off the press’ proposed model of integrated support for the regions where all stakeholders, but especially residents, will be better off.
Key themes included integration and consistency, especially in communication and documentation, better access to clinicians’ tools and resources, improving workforce capability and capacity and better anticipatory planning and practice.
The model includes a roadmap to get this work underway and every stakeholder has a role to play.
What’s so pleasing is that everyone that participated was keen to work together and whilst the hard work is only just beginning – we know there is passion and commitment to doing better.
