The Mid North Coast Local Health District’s High Risk Foot Service provides a multidisciplinary, coordinated approach to managing foot complications.
The service aims to prevent unnecessary presentations to hospital, admissions and possible amputation.
The criteria for referral includes any person with an infected or probe to the bone wound of the foot, or has an acute Charcot neuroarthropathy and may also have:
- Diabetes
- Previous amputation
- Peripheral neuropathy
- Peripheral arterial disease
- Peripheral vascular disease
- Malnutrition
- Osteomyelitis
The patient will be provided with access to a diabetes educator, dietitian, podiatry, wound care nursing (and social worker in Coffs Harbour), including education and treatment on initial assessment. Clients who are referred to the service are to be made aware that they will be reviewed by a multidisciplinary team and are expected to be proactive with the care and advice provided.
A report will be provided back to the GP following each session. This will include findings and recommendations from each of the health providers involved.
These services are now accepting referrals from across the Mid North Coast.
