The Mid and North Coast HealthPathways team are inviting expressions of interest to participate in a series of Healthy Ageing and Palliative Care pathway workgroups in 2024/25.
Two workgroups will be assembled to support the development of five new HealthPathways aimed at supporting practice nurses, residential aged care nurses and general practitioners when providing care to older people or those receiving palliative care. The pathways will address:
- Early intervention for healthy ageing
- Living with dementia
- Multimorbidity
- Early intervention in palliative care
- Anticipatory prescribing and end-of-life care.
The HealthPathways are being developed as part of a larger collaborative project funded by Healthy North Coast under the Ageing Strategy which seek to build capacity within general practice to deliver nurse-led care to older people and those living with chronic disease.
The workgroups and resources developed will cover clinical aspects of care relevant for nurses and GPs and provide practical aspects of implementing nurse-led appointments or clinics within a general practice setting, including practice support and funding options.
Expressions of interest are invited from practice nurses, general practitioners, residential aged care nurses, practice managers and other specialist nurses or allied health professionals with an interest in healthy ageing or palliative care.
Workgroups will be held online or face-to-face, depending on membership, for approx. 90min. There is an anticipated commitment of 2-3 workgroup meetings for initial pathway development, and further feedback via email. Additional workgroups will be held over 2024/25 to complete the suite of pathways and facilitate pathway review following a pilot of implementation. Workgroup dates and times are TBC with an expected start date of early February 2024.
Participation in the workgroups and related activities will be reimbursed according to the Healthy North Coast Stakeholder Remuneration Policy.
