Healthy North Coast is seeking expressions of interest from general practices interested in establishing a COVID Care Home Visit Service.
A starter package of up to $7000 may be offered to 2 lead practices per local government area that can commit to supporting home visits.
Healthy North Coast’s COVID Care Home Visiting Service will help support and manage COVID-19-positive patients within the community and may help avoid unnecessary escalation of patients to hospital.
COVID Care Home Visiting can support patients including people in palliative care, caring roles, antenatal settings and those requiring wound care. The service may also include face-to-face visits for patients in residential aged care facilities and may assist in management of outbreaks.
Start-up funding package
Under the start-up funding package, up to $2000 for nursing / clinical care (up to a 40-hour package) and $5000 for clinical equipment and technology to establish a dedicated mobile service for a home visit is available. This may include:
- devices to support point of care documentation
- VPNs for remote connectivity to clinical information systems
- dedicated portable blood pressure, vitals and home monitoring devices
- peak flow and mobile spirometry machines.
