Healthy North Coast is inviting GPs with a special interest, expertise or qualifications, who are prepared to receive patient referrals from colleagues, to register their details as part of the GP to GP referrals page on HealthPathways.
This resource aims to increase connectivity and collaboration within the local health workforce and give GPs the opportunity to make better use of their skills and specialised knowledge.
The page provides a list of GPs who are happy to receive referrals from other GPs to see patients in their special area of interest. To date, over 90 GPs have registered.
As an example, a GP might need to find someone else to provide S100 prescribing for HIV, or medical termination of pregnancy. The GPs accepting referrals are only managing their area of interest, and usual GP care will remain the responsibility of the original GP.
To register, complete the GP Referral registration form on our website. There is a free text option on the form that can be used to fill in areas of interest, if not already listed.
Included on the GP to GP referrals page is a framework of principles for the process that covers both the sending and receiving of referrals, as well as the scope of consultations and investigations, and requirements for closing the referral loop.
