Care finder services have now been added to Mid and North Coast HealthPathways.
Do you have older patients who need intensive support to access aged care and other services and supports? This may include people who:
- are homeless, or at risk of homelessness
- are isolated and without family or other supports
- have communication barriers
- haver difficulty processing information to make decisions or, due to previous negative experiences, are hesitant to engage with aged care services.
Care finders are a commissioned service through Healthy North Coast to support vulnerable older people who would not be able to arrange services without intensive support and do not have a family member or friend who can help.
Care finders can help people understand what aged care services are available, set up an assessment and find and choose services. They can also help older people to access other community supports. They can help with both accessing services for the first time and changing or finding new services and supports.
On the North Coast, four organisations are funded to provide this important free service in various locations: EACH, Carexcell, Lifetime Connect and Footprints Community.
HealthPathways contains information on how to assess, manage and refer in the local context of available services. HealthPathways can support clinicians to navigate the local health system and easily check referral requirements, helping ensure patients receive the right care, in the right place, at the right time.

The pathway also includes information about types of services available, and information about how to access urgent respite care or community nursing, and patient information handouts.
For older people who may need less intensive support to access My Aged Care services, the HealthPathway also lists information about Aged Care Specialist officers, who provide face-to-face help in 70 Services Australia service centres.
