The health of high-risk patients had health professionals and community members united to develop key strategies at the latest co-design workshop in Port Macquarie.
Health professionals and community members discussed the issues surrounding the seasonal demand surges facing our local health system in a workshop held in Port Macquarie recently. The focus of the workshop was around how to keep high-risk patients healthy and in their homes for longer, and the effective sharing of health data.
Over 40 representatives from MNC Local Health District, NCPHN, general practices, other primary health care providers and the community provided valuable insight into the challenges that are being faced. The group worked together to develop potential strategies that will provide the basis for further analysis on how to overcome these challenges.
This workshop is one of a series of co-design events being held across the Mid North Coast. Aimed at capturing a range of ideas and experiences, these workshops provide a forum for discussion and information sharing that will drive the continual improvement of our health system.
