The new Renal Supportive Care HealthPathway was published in December 2018 and developed by the HealthPathways team in liaison with the Northern NSW and Mid North Coast Renal Managers.
Patients referred and involved in the service remain under the care of their nephrologist and referring general practitioner.
Renal supportive care is available to patients with end-stage renal disease (stage 4 and 5) who:
- Choose not to have dialysis or transplantation as a treatment option.
- Are already on a renal replacement therapy and are experiencing new comorbidities and/or uncontrolled symptoms or renal failure, or
wish to withdraw from dialysis.
The service is a consultancy speciality using a multidisciplinary approach, involving two outreach teams comprised of a CNS2, dietitian and social worker for assessment and management for issues affecting the patient’s quality of life.
The Northern NSW Local Health District has two teams based at Lismore Base Hospital and Pottsville Health One. The Mid North Coast Local Health District also has two teams, based at Port Macquarie and Coffs Harbour Base Hospitals. The teams conduct outpatient clinics at health facilities throughout their networks and conduct home visits for unwell patient unable to travel. Phone and telehealth contact with clients can be arranged where appropriate.
The pathway provides clear referral guidelines and options, which list details on access criteria and information required by the referring practitioner about the patient. Each individual service lists location, description, contact details and any specific information related that service.
