Soon you and your patients will be able to view Pharmacist Shared Medicines Lists (PSML) that have been uploaded to a My Health Record. This is a list of medicines a consumer is known to be taking at the time the list is created, including prescribed, over-the-counter and complementary medicines.
A Pharmacist Shared Medicines List could be created by:
- A pharmacist after completing an in-pharmacy medicine review.
- A pharmacist in preparing a dose administration aid.
- A hospital pharmacy for patients when they are discharged.
A Pharmacist Shared Medicines List can benefit patients with complex conditions or chronic diseases who are taking multiple medications.
A link to the most recent PSML will be provided in the Medicines Information view.
The PSML will improve information sharing between health professionals, ensure continuity of medicine management for patients who transfer between care settings, and provide timely access to a curated medicines list. It will also help to reduce medication interactions, which will help to improve both medicine safety and patient safety. For example, in reconciling medicines for a PSML, a pharmacist may identify a drug interaction between a complementary medicine (such as St John’s Wort) and a prescription medicine (such as an antidepressant) and take action to resolve this.
Only a pharmacist can prepare and upload a PSML to a patient’s My Health Record at this stage.

This live and interactive webinar will explain what the PSML is and outline the benefits to you and your patients in accessing this information.
