Healthy North Coast has partnered with the Agency for Clinical Innovation and Mid North Coast Local Health District to improve wound management across the Mid North Coast community.
The project aims to enhance the integration of chronic wound management services across all care settings and disciplines by strengthening partnerships, building knowledge about wound management HealthPathways, and improving local referral and escalation pathways.
The project team held workshops in December 2023 to guide the development of project activities. Three priority areas were identified for action, including:
- promoting and updating HealthPathways wound care and referral pathways
- forming a Community of Practice (CoP) to build expertise and improve wound care across the region
- establishing a working group to determine wound care capabilities and education in Residential Aged Care Home (RACH) settings.
HealthPathways and wound care
HealthPathways has comprehensive clinical and referral information about a range of wound types, including hard to heal wounds, skin tears, burns injuries and pressure injury in spinal cord injury, as well as a useful new resource page listing wound management products.

Aged care wound care capabilities and education
A working group has been recruited to identify which wound care capabilities apply to staff in aged care, and to provide advice on education session development.
