Over the last two years, the Northern NSW Local Health District (NNSW LHD) and NCPHN have been running a Shared Care Planning tool project in Northern NSW with support from eHealth NSW and the NSW Ministry of Health.
The proof of concept project had a set timeframe of two years to test the “Shared Care Planning” model of care, which is a priority for the Commonwealth and NSW Health. The project was also tasked with evaluating the system from the vendor, Orion Health, for eHealth NSW as a potential statewide tool.
With funding for the project ceasing on 30 June 2019, the the Shared Care Planning project will close on this date.

NNSW LHD will provide a comprehensive evaluation package to eHealth NSW and the NSW Ministry of Health. This will help inform the strategic direction of Shared Care Planning in NSW and Australia.
This has been an innovative project in NNSW and has received a lot of visibility in eHealth NSW and the Ministry of Health. The evaluation will improve digital solutions to support better connection between NNSW LHD and general practices.
Both NNSW LHD and the NCPHN will continue to work together to attract new opportunities from eHealth NSW, the NSW Ministry of Health and the Australian Digital Health Agency, including for projects like eReferrals, Secure Messaging and other Shared Care Management tools.
