Northern NSW Local Health District (NNSWLHD) has now gone live with their mental health admission and discharge notifications (ADNs). This is an extension to their existing ADN services, with ADNs previously being delivered for chronic disease patients.
Hospital admission and discharge notifications are intended to improve the flow of information to the GP about a patient’s health journey beyond primary care.
Hospital ADNs benefits your patient and your practice
- Timely admission notifications can to help alleviate previous patient safety concerns.
- Admission notifications provide the opportunity for you to forward current health summary and medication list to the ward to support appropriate treatment, or to contact the hospital to provide collateral information for admission and discharge.
- Discharge notifications alert you to the need for possible post-discharge appointments before a patient contacts your practice.
- Discharge notifications tell you to expect a follow-up discharge summary.
What does the ADN look like?
Below is an example of how the ADN will look, which may slightly differ depending on your clinical software. It will state if it is an admission or discharge notification, and will include the ward in which the patient was admitted or discharged from.
