Supporting residents to live and die well starts with access to clear, local and compassionate information.
As part of Healthy North Coast’s ongoing work to support Residential Aged Care Homes (RACHs), 2 new palliative care resources are now available: a Palliative Care Fact Sheet and a Palliative Care on the North Coast Booklet.
Designed to support residents, families and the staff who care for them, these informative resources explain what palliative care is, how it fits within the residential aged care context, and the local services available across the NSW North Coast. They are intended to support early, informed conversations, reduce uncertainty, and help ensure care aligns with residents’ values and wishes.
Together, the resources provide:
- clear, plain‑language explanations of palliative care
- information on local services and supports
- practical guidance for planning ahead
- details on Advance Care Planning and Advance Care Directives
- supportive information for families and carers at different stages of illness
Developed in collaboration with local health professionals, the resources reflect the realities of residential aged care, support early identification of palliative care needs, and align with the new Aged Care – End of Life Pathway.
The resources can help RACH teams:
- guide conversations with residents and families
- support Advance Care Planning discussions
- provide consistent evidence‑based information
- reduce avoidable hospital transfers by helping residents remain where they feel most comfortable
- support new and casual staff to better understand local pathways and services.
Why it matters
For many older people, residential aged care is home. Clear, accessible information supports confidence, reduces distress, and enables a more person‑centred approach to end‑of‑life care, for all involved.
