Healthy North Coast has released two new resources – a Palliative Care Fact Sheet and a comprehensive Palliative Care on the North Coast Booklet – to support health professionals in guiding patients and carers through palliative and end-of-life care.
Developed by HNC’s Healthy Living & Ageing team, these resources provide clear, localised information to help improve understanding of palliative care, streamline navigation across the region, and assist patients to plan ahead with confidence.
These tools are designed to make it easier for clinicians to start early conversations, reduce uncertainty for families, and connect people to the right supports at the right time.

- Practical guidance for patients and carers
- Advance care planning prompts and explanations
- Carer support, including financial and practical considerations
- Clear, accessible language for easy use in consultations.
How to use these resources in practice:
- Provide printed copies to patients and carers during relevant consultations
- Incorporate the booklet into care planning discussions
- Share via practice newsletters or waiting room displays
- Use the Fact Sheet as a quick reference tool to initiate ACP conversations.
