Healthy North Coast is seeking a suitably qualified evaluation partner to help shape the future of community-led primary care innovation across the region.
This provider will have the opportunity to undertake a comprehensive evaluation of the Primary Care Impact Grants program.
For over 12 months, the Primary Care Impact Grants have supported a wide range of locally designed, multidisciplinary projects aimed at strengthening chronic disease prevention and improving the way care is delivered in primary care settings.
Unlike larger, condition-specific or geographically targeted funding models used in other regions, Healthy North Coast has intentionally adopted a small-grant, flexible commissioning approach designed to empower providers to respond directly to their community’s needs.
Primary care engagement with the PCI grants program has been high, with 94 applications received in the first 3 rounds. 48 applications were successful, with providers across the region being awarded grants to address a variety of chronic conditions or risk factors.
What the evaluation will explore
The successful evaluation partner will assess the program across several key areas:
- PCI Grant design, evaluation and awarding processes
- Local health outcomes and health system impacts
- Insights from the unique commissioning method
- Recommendations for the future of the PCI Grants

The Primary Care Impact Grants program represents a bold shift toward community-driven innovation in primary care. By evaluating what worked well, what could be improved and what lessons can be carried forward, Healthy North Coast aims to strengthen future commissioning and ensure local providers are supported to deliver the right care, at the right time, for their communities.
