The Mid North Coast Local Health District (MNCLHD) is asking GPs to ensure their contact details are up to date.
This enables patient information, such as electronic discharge summaries and admission discharge notifications, to be sent via electronic communication – the main communication mechanism between hospitals and primary healthcare providers.
Accurate GP details are critical for continuity of care and to ensure well-coordinated, safe and effective clinical handover. This is a known risk area for patient harm, particularly in the transition from acute care to primary care.
Updating GP details through a webform on the MNCLHD website enables timely delivery of important information, an area that a majority of GPs in a 2017 survey identified as needing streamlining.
Since the 2018 establishment of the webform, the percentage of electronic discharge summaries delivered to GPs via secure messaging increased from around 69% in 2019 to 83.6% in 2020.
The webform also allows GP details to be updated on the MNCLHD Electronic Medical Record. It supports ongoing improvements to the sharing of information from MNCLHD to primary practitioners.
