Partnering with aged care teams to provide quality care for residents
This month, we’re proud to introduce Vanessa Gee, our lead on a number of projects that support aged care teams to give residents the best possible care. This includes palliative and end of life care and improving solutions for care ‘after hours.’
Ness says:
‘With over 30 years of experience in the health sector – as a registered nurse, clinical manager, and program lead – I understand the unique challenges that aged care homes face when supporting residents through their final stages of life.
‘I‘m here to work with our aged care providers to enhance education and clinical support. This includes helping teams use tools such as the HNC Deteriorating Resident Triage Tool, advance care planning, and anticipatory care strategies.
‘My focus is on building stronger connections across health services to support the vital work you do in caring for your residents.
‘When I’m not at work, you will find me on our macadamia farm, hopefully in the garden, avoiding the mowing or enjoying a walk with my Labrador, Champ. If you’re interested in accessing education or resources, I’d be very happy to connect.’
