One of the reasons patient information (discharge summaries) isn’t received by some GPs is because the hospitals are unaware that contact details have changed or there is uncertainty as to which details are still correct.
Whenever a GP or practice details change – or if you suspect that hospitals have to choose from multiple contacts – there’s now a simple way to make sure the hospital is using your most recent contact information.
In one easy step, practice managers or GPs can be sure that the hospital has the most recent GP contact details. Simply use this form.
This form triggers hospital teams to urgently upload the new information to the regional hospital system so you can receive information about all of your patients who identify as being under your care.
