Background
In September 2024, Healthy North Coast proudly launched the Healthdirect Fast Track After-Hours service. As one of only three PHNs chosen to partner with Healthdirect and able to provide this after-hours service to all Aged Care Homes across our region. We have been closely collaborating with Healthdirect to monitor usage and ensure your concerns, barriers and successes are heard.
Now, Healthdirect are conducting a six-month evaluation of the service and they want to hear directly from you!
- What are your experiences with the service? Did you receive a callback? Were the needs of the resident and home met?
- What barriers have you encountered? Issues like the inability for service to chart medications or challenges in implementing care advice from Healthdirect GPs?
