The Mid North Coast Local Health District’s Admission and Discharge Notification (ADN) pilot went live on 29 September 2020. The project has delivered over 84,213* notifications to patients’ nominated GPs.
ADNs are intended to improve the flow of information a GP receives about a patient’s health journey.
Initial feedback on the pilot has been positive.
This is amazing! Thank you, thank you, thank you for making this happen!!!!! It just streamlines the whole process, it's amazing.
Natalie Young, Practice Manager, Scotts Head Medical
I received my first notice today of admission and I think it was succinct and to the point.

Now it’s your turn to let the pilot know how you’re responding to these notifications.
This short, seven-question survey will take you less than three minutes to complete.
Your feedback will help to improve the notifications you receive about your patients in the local health system. Please encourage GPs in your practice to participate — together, we can continue to improve the safe transfer of care for our patients.
