The Heart Foundation is inviting general practices to register their interest in participating in the 2022 National Heart Health Check Recall Program.
The program builds on the national pilot implemented in 2021, which supported general practices to send out an SMS recall message to patients for a heart health check. The pilot reached more than 42,000 eligible Australians.
Known as ‘Text to Detect’, this will be the largest quality improvement program of its kind in Australian primary care.
The Heart Foundation is offering 200 practices the opportunity to participate in phase 2 of Text to Detect. Register your interest here.
The Heart Foundation has also developed a Heart Health Check Toolkit to help general practices integrate the Heart Health Check into routine patient care.
