The End of Life Directions for Aged Care (ELDAC) specialist palliative care and advance care planning advisory service has a range of resources available to help primary care professionals.
These aim to support providers and give them the confidence and tools to deliver advance care planning and palliative care.
The ELDAC Primary Care Toolkit resources are across clinical action, team action and HealthPathways, including referral paths and best practice management of conditions.
