Do you see older patients who ideally need aged care services and supports to stay at home but struggle to navigate My Aged Care for the care they need?
Healthy North Coast is delighted to have extended the Care finders program for another 4 years, providing free, intensive support to help older patients who are eligible for Commonwealth-funded aged care to get services, even in the trickiest of circumstances.
Who can care finders help?
Generally, people eligible for care finder support have no carer, family or friends to support them. They may be socially isolated, face communication challenges or feel reluctant to engage with government services. They may also be:
- Individuals who are financially or socially disadvantaged
- People experiencing homelessness or who are at risk of homelessness
- Residents of rural or remote communities
- Aboriginal and Torres Strait Islander peoples
- People from culturally and linguistically diverse (CALD) backgrounds
- Individuals affected by past forced adoption or child removal practices
- Care leavers
- Veterans
- Members of the LGBTQ+ community
What can care finders do?
- Speak with My Aged Care on the clients behalf
- Attend assessments to provide guidance and advocacy
- Help compare and choose aged care providers
- Assist with completing forms and understanding service agreements
- Follow up once services are in place to ensure they’re meeting client needs
- Connect people with health, mental health, housing and community groups.
How can you refer patients to care finders?
Referrals can be made at any time, with the person’s consent, by GPs, allied health practitioners, social workers, hospital discharge planners, aged care assessors, and carers. Older people can also self-refer if they need additional support.
On the North Coast, Care Finder services are delivered by:
