Residents of Residential Aged Care Homes (RACHs) across the Mid North Coast Local Health District (MNCLHD) will now have improved access to timely, specialised care thanks to the launch of the new Aged Care Outreach Service (ACOS).
The service, delivered by Mid North Coast Virtual Care, is designed to support residents who become acutely unwell or deteriorate, helping to reduce unnecessary transfers to hospital and emergency departments (EDs).
Through a mix of virtual consultations and face-to-face reviews, ACOS provides rapid clinical assessment, medical oversight, and coordinated care planning in partnership with the resident, facility staff, GPs and families.
The service has commenced at 11 pilot sites across the MNCLHD, ensuring early testing and refinement of referral pathways. In addition, a significant milestone began from 1 September 2025, when NSW Ambulance paramedics will be able to refer residents directly to ACOS from a further 17 Residential Aged Care Homes that have opted in to the program.
This direct referral pathway means that when paramedics are called to a participating facility for a non-life-threatening condition, they can connect residents straight to ACOS for medical assessment and treatment.
This new option not only supports residents to remain in the comfort of their familiar environment, but also ensures that hospital resources remain available for those with more urgent needs.
MNCLHD Chief Executive Jill Wong said the implementation of ACOS reflects a strong commitment to person-centred, innovative models of care.
“We know that hospital transfers can be stressful and disorienting for aged care residents. ACOS provides the right care, at the right time, in the right place. By working closely with facility staff, GPs, and NSW Ambulance, we are delivering safer, more coordinated care and improving outcomes for our older community members.”
Operating 7 days a week, including public holidays, ACOS is supported by a dedicated team of Registered Nurses, Nurse Practitioners, Medical Officers, and a specialist Geriatrician. The service is part of MNCLHD’s broader Out of Hospital Care strategy, which focuses on delivering safe, sustainable, and accessible care closer to home.
