What is the ACOP program?
Aged care providers can receive government funding to engage an on-site pharmacist (Aged Care On-site Pharmacist or ACOP) to work on site within their homes.
Part of the clinical team, ACOP’s offer extra support and guidance on resident’s medications and the quality use of medicines.
Healthy North Coast is working to assist aged care providers interested in this program with advice on eligibility, what’s on offer and how to go about hiring an on-site pharmacist.
Why is greater involvement from a pharmacist important?
The Royal Commission into Aged Care Quality and Safety identified the management of medications as a key issue for resident safety and quality of care, recommending that pharmacists become part of the residential aged care team.
Having an on-site pharmacist embedded in the home as part of the clinical care team is significant for the prevention of medication misadventure.
Who can have an ACOP?
Open to any Aged Care Homes that has an electronic National Residential Medication Chart (eNRMC) in place, or is committed to introducing an eNRMC within the next 12 months.
The funding provides for one on-site pharmacist per 250 beds, or blocks of one day per 50 beds . This enables on-site pharmacists to have a regular presence on site, even in smaller residential aged care homes.
There are a few exclusions to be aware of and where an on-site pharmacist is not engaged, funding for a visiting pharmacist under the QUM and RMMR Pharmacy Programs will continue.
You can find further information at Department’s Aged Care On-site Pharmacist Measure webpage. Under ‘Learn More’ there is a guide on the ACOP Measure for pharmacists and RACHs.

Stories of success
Here is what Owen Lednor, General Manager of Shoreline at Coffs Harbour had to say:
“Having an on-site pharmacist is a game-changer for both our clinical team and our residents. Staff have found that the ability to collaborate in real-time with a dedicated pharmacist has significantly improved medication safety, streamlined communication between clinical staff and GPs, and improved overall health outcomes.”
Owen reports that the pharmacist’s presence means Shoreline staff and medical team can quickly clarify prescriptions, adjust dosages, and proactively manage polypharmacy risks.
This has reduced delays and improved the confidence of the team when making decisions. A GP who works regularly with the ACOP at Shoreline said “It makes my job easier.”
