The aim of the Kidney Health Project is to develop an increased focus on early identification and interventions to achieve better kidney health, thus preventing the onset of chronic kidney disease.
The Kidney Health Project will use the following strategies to achieve this:
- A population wide campaign about kidney health utilising health literacy principles and considering cultural appropriateness.
- Development and documentation of resources to support early identification and interventions to achieve better kidney health in primary care settings and to build the capacity of primary health workers to increase the education and awareness of early warning signs of CKD. This includes the development of a ‘Measuring for Improvement’ Guide to support continuous quality improvement.
- An initial pilot of the project across 5 northern AMSs, in collaboration with the NNSW Aboriginal Health Partnership and with guidance from Graeme Turner, NNSWLHD Clinical Nurse Practitioner.
