Compared to the general population, people with intellectual disability experience:
People with intellectual disability can get a health assessment once every year. The health assessment is supported by Medicare.
This health assessment is important. It can help the person with intellectual disability, their families and supporters, and doctors to better understand the person’s health. It also helps the person to access follow up services to identify and treat health issues.
Data that shows only 7% of people with intellectual disability in Australia are visiting a GP for their annual health assessment.
Increase uptake of annual health assessments by people with intellectual disability
Provide more comprehensive health plans for people with intellectual disability that include:
– action on health promotion and disease prevention
– improved disease detection and management
Rate of patients with an intellectual impairment receiving an annual health assessment
Rate of use of the Comprehensive Health Assessment Program (CHAP) tool in annual health assessments
It’s Doctor Time!
It’s Doctor Time! has been developed by Inclusion Australia in response to new data that shows only 7% of people with intellectual disability in Australia are visiting a GP for their Medicare-supported Annual Health Assessment. Comprehensive Health Assessment Program (CHAP)
The Comprehensive Health Assessment Program (CHAP) is an evidence-based tool for conducting annual health assessments for people with intellectual disability in Australia. There are free versions of the CHAP available for adults 18 years and over, and young people aged 12 to 18 years.
