The Winter Strategy program was run in the northern half of NCPHN’s footprint during 2017 and 2018. The program aimed to deliver improved care coordination, self-management support and access to general practice for patients at high risk of being particularly unwell through the flu season. It also supported provision of needed allied health services and collaboration with the Local Health District’s (LHD) Chronic Disease Management teams. It has been a combined PHN/General Practice/LHD initiative.
The program includes patient and clinician experience measures, sick day action plans, and improving the collaboration between LHD and private services; all strengthening the integration of the health system.
