NCPHN is now seeking proposals from experienced providers to deliver an Information, Assessment and Referral Service to ensure people with mental health or alcohol and other drug support needs are able to access services appropriate to their level of need across the North Coast.
What is an Information Assessment and Referral Service?
This service is a first point of call for individuals, service providers and clinical practitioners to identify and connect to services that are available for people with mental health or alcohol and other drug support needs. The service will receive enquiries, assess an individual’s situation and facilitate referrals to the most appropriate service.
The successful provider will partner with NCPHN in the implementation of the stepped care model, ensuring that a person is provided with the right level of care at the right time and, where necessary, supporting a person’s transition through different levels of care, increasing or decreasing in intensity as appropriate.
Who can access it?
Anyone seeking information or connection to the services that operate within the NCPHN region. NCPHN anticipates that initially the service will receive most of its referrals from General Practitioners. However, community members, their carers and other service providers will also be encouraged to use this service to identify appropriate services.
