The Mid North Coast Local Health District’s palliative care team has been bolstered by the addition of a new CNC2 position based in the Coffs Harbour Clinical Network.
The position, which is being shared by Scott Handsaker and Susan Gillies, is to support patients with end-stage cardiac and respiratory disease. It has been funded for the next two years as part of Flexible Funding.
Scott and Susan are excited to take on the new role which will provide consultation to patients with end-stage cardiac and respiratory disease.
The pair will initiate early intervention towards a supportive approach, assessing symptoms, optimising medications and management plans, providing a holistic plan of care tailored to meet the needs of patients and their carers. They will also initiate advance care directives, establishing preferred place of care, integrate with health providers and assess for ongoing services where required.
Scott and Susan will also follow up patients in the community following discharge, to ensure their discharge plan is understood and optimised and a management plan is established. The expected outcome of this will be a reduction in hospital readmission. The CNS2 has the ability to establish palliative care medical clinics for these clients.
This position will wrap around and support established chronic care, connecting care, and palliative care services as well as working with discharge planners and inpatient teams to facilitate discharges from hospital.
