I recently started as the Integrated Care Program Nurse Educator at the Northern NSW Local Health District (NNSW LHD). This position is a collaboration between North Coast Primary Health Network and NNSW LHD.
The focus of the project is on building the skills and capacity of practice nurses and nurses working in aged care to intervene early in a patient’s deterioration – with the ultimate goal of preventing hospitalisation.
This position was set up to support you and your professional development goals. The project is running until October 2018 and the position is full time.
I started the project by analysing primary health care nurse needs (general practice and RACFs), and am now working to narrow the scope and priority of education to be developed and implemented in 2018. I will be looking at professional development opportunities to support patient centred care and am looking forward to collaborating with the Primary Care Nurse Networks.
I have so far collaborated with LHD staff, Residential Aged Care facility staff and accessed data from the PHN. I would greatly appreciate hearing from you!
