The Mid and North Coast HealthPathways patient info website is now live.
What is it?
patient info is a public website for patients to find reputable information on health conditions and local health services. The resources listed are taken from the ‘Patient Information’ sections of the relevant HealthPathways clinical page.
The site supports GPs to educate their patients and improve health literacy
by providing a range of peer-reviewed health information and resources.
patient info also contains a range of localised fact sheets which are adapted from the HealthPathways clinical content.
How does this benefit my patients?
In practical terms, a GP is not able to provide multiple sources of patient information within a standard consultation. Directing patients to patient info, empowers them to download relevant, high-quality resources as needed.
GPs can be confident the resources provided are reputable as they were selected by local GPs and hospital specialists during the HealthPathways localisation process.
How do I direct my patients to the site?
The patient info site can be found at:Â http://www.mancpatientinfo.org.au/
To direct patients to patient info, use the cards provided and write the name of the patient’s health condition as well as any other notes.
