Thank you for taking part in the Integrated Team Care (ITC) program client experience survey. We are interested in hearing from community about their experiences with ITC service providers, and other specialist and allied health services.
By sharing your experiences about accessing the ITC program, you’re helping us improve health programs and services for your community.
Who's conducting the survey?
This survey is conducted by Healthy North Coast and Aboriginal Medical Services. Your valuable feedback will contribute to program improvements.

What you will be asked to do
You will be asked to participate in an anonymous survey about your experiences with ITC service providers, specialists, and other health services. The survey findings will be used to evaluate and improve the ITC program.
Participation prize
You can opt in to participate in a prize draw for completing the survey. There will be four $50 grocery vouchers randomly drawn each 6 months. Name and phone number is required.
Confidentiality and privacy
This survey is completely confidential, no personal information will be shared or published.
You can opt to complete the survey confidentially, or you can provide your name and phone number. Providing your name and phone number allows participation in a prize draw.
Participant prize conditions
1. Randomly drawn 8 x $50 cashcards every 12 months
2. Open to residents of NSW North Coast communities living in the following LGAs (Ballina, Bellingen, Byron, Clarence Valley, Coffs Harbour, Kempsey, Kyogle, Lismore, Nambucca Valley, Port Macquarie, Richmond Valley, Tweed)
3. Participants of the ITC Program
4. Aged 18+ years
5. Complete the ITC Client Experience Survey
6. Complete the prize draw nomination at the end of the survey
