About the survey
We’re inviting people aged over 55, to share their experiences with health care. Your feedback will help us understand what’s working well and where improvements are needed.
All responses are anonymous and confidential, and the findings will be shared in a final report on the Healthy North Coast website.
Surveys close on the 30 September 2025 at 5pm AEST.

By taking part in the Over 55 Survey, you can enter the draw to win one of three $100 gift cards. The prize draw is open to residents of NSW North Coast communities living in the following LGAs: Ballina, Bellingen, Byron, Clarence Valley, Coffs Harbour, Kempsey, Kyogle, Lismore, Nambucca, Port Macquarie, Richmond Valley, and Tweed. To be eligible, participants must be aged 18 years or older and cannot be employees of Healthy North Coast. To enter, you must complete the survey and submit the prize draw nomination form.
