Better Health Prize Draw Terms and Conditions

For all participants:

1. There will be 20 x $100 cashcards randomly drawn 
2. The draw is open to residents of NSW north coast communities living in the following LGAs (Ballina, Bellingen, Byron, Clarence Valley, Coffs Harbour, Kempsey, Kyogle, Lismore, Nambucca, Port Macquarie, Richmond Valley, Tweed)
3. The draw is not open to employees of Healthy North Coast
4. You must be aged 18+ yrs to enter
5. You must complete the Better Health survey
6. You must complete the prize draw nomination form
