North Coast Primary Health Network (NCPHN) is recruiting clinical champions to assist in our Patient Centred Medical Home (PCMH) program. We are looking for General Practitioners with an interest in quality improvement and patient centred care. PCMH clinical champions will be remunerated as per the NCPHN’s Stakeholder Engagement Remuneration Policy (currently $115 per hour).
The Patient Centred Medical Home model is proposed as a model for “transforming primary care and improving efficiency and effectiveness in the health care system” (Patient Centred Primary Care Collaborative 2015, cited in ACI, 2016).
The PCMH model is centred on the patient and their needs, with health and community services coming together to provide a connected and integrated approach to care. The benefits of this model are well documented and include: a holistic approach rather than being disease or organ specific; care that is delivered closer to the patient’s home; care based on long-term relationships; care focussed on patient self-management where the patient is an active participant; care which is preventative and proactive; and care which is less costly to provide.
NCPHN works with general practices and Aboriginal Medical Services, using a quality improvement approach, to assist them to transition to medical homes. This approach includes completing PCMH self-assessments with practices and facilitating workshops with individual practices to discuss their self-assessment results and to develop improvement ideas to progress their transition to a medical home.
There has been so much interest in this program that we now require clinical champions to assist with the roll-out. The role of the clinical champion is to co-facilitate the PCMH practice workshops. The workshops are usually held on weekday evenings. Training on the PCMH model and facilitation of the workshops will be provided. The time commitment is one to two workshops per month.
