One year on from the official launch of caring@home, health professionals and carers have reported how valuable the resources are in assisting home-based patients and their care team with end-of-life symptom control.
More than 3,300 caring@home packages for carers have been distributed to specialist palliative care services, domiciliary nursing services, rural and remote health services and GPs nationwide to assist patients and their carers.
The resources are having a tremendously positive impact on patients, their carers and their wider healthcare teams, reducing patient distress and unwanted hospital admissions at end of life.
