The Mid North Coast Local Health District (MNCLHD) and NCPHN are collaborating on a joint project to support general practice in responding to the surge in healthcare demand over winter. The Seasonal Demand Coordinated High Intensity Patient Care Initiative aims to empower patients with chronic and/or complex conditions to stay well and safely at home during winter.
The initiative has accepted enrolments from the following general practices to provide this opportunity to 180 patients:
- Dr Lerm’s Surgery Woolgoolga
- Nambucca Healthcare Centre
- Urunga Medical Centre
- Westside Medical Centre Port Macquarie
NCPHN will provide these general practices with funding for up to 720 Practice Nurse hours and resources to identify and support high intensity patients over winter with an emphasis on:
- Priority access to general practice
- Active engagement with patients via patient focused care team coordination and early intervention when health deteriorates
- Increased self-management through Sick Day Action Planning and other health coaching
Participating practices are currently identifying patients to participate in collaboration with the MNCLHD Integrated Chronic Disease Management team.
