Supports Aboriginal people through culturally appropriate mental health and wellbeing services delivered through local Aboriginal community-controlled health organisations.
Provides creative arts, music therapy and cultural therapy to support people with moderate to severe mental health challenges.
Specialists deliver education and patient case conferences within general practices, building clinician capability, strengthening specialist-GP collaboration, and improving chronic disease outcomes.
The Chronic Disease Prevention Grants Program funds local, evidence-based initiatives that prevent chronic disease, improve health outcomes, reduce inequity and support healthier North Coast communiti
Funds multidisciplinary healthcare teams in primary care to improve chronic disease management, increase access to coordinated care, improve quality of life and reduce avoidable hospitalisations.
