Older people’s health
Supporting people to stay healthy, independent and connected as they age is an important part of building healthier communities across the North Coast.
Healthy North Coast works with primary care, aged care providers, health services and community organisations to improve access to health care and support for older people. Our focus includes prevention and healthy ageing, stronger connections to local services, and helping people receive coordinated care that responds to their needs.
Supporting healthy ageing on the North Coast
Good health as we age is about more than managing illness. Staying active, socially connected and involved in the community can make an important contribution to physical health, mental wellbeing and independence.
We support local initiatives that help older people connect with services, activities and opportunities in their communities, while working across the health system to improve access to appropriate care and support.
Older people’s health on the North Coast
Our Health Needs Assessment (HNA) helps us understand the health and wellbeing needs of older people across the North Coast, including where there are gaps in services, barriers to accessing care and opportunities to improve health outcomes.
These insights help guide how we plan, prioritise and commission services, and work with our partners to respond to the changing needs of an ageing population.
Healthy Towns
Building healthier, more connected communities
Healthy Towns brings communities together to identify local health and wellbeing priorities and develop practical solutions that reflect the needs and strengths of each community.
The initiative supports locally led approaches to healthier living, connection and wellbeing, including opportunities that can help people remain active and engaged as they age.
Local Connector
